Conditions treated

Is your condition on this list?
You may not need to wait two years.

The telemedicine consultation covers your imaging, your diagnosis, and whether surgery is indicated — and whether the procedure you’ve been recommended is truly the right one for your anatomy. Many Ontario patients arrive having been told fusion is their only option. For a significant proportion, it is not.

Lumbar Disc Herniation & Sciatica
Leg pain, numbness, or weakness from a herniated lumbar disc pressing on nerve roots. Often surgical when conservative treatment has failed.
Common — get off the waitlist
Cervical Disc Herniation & Radiculopathy
Arm pain, weakness, or tingling from a cervical disc compressing a nerve root. Disc replacement often preferred over fusion — ask whether you were offered it.
Ask about disc replacement first
Cervical Myelopathy
Spinal cord compression causing hand weakness, balance problems, or gait changes. Time-sensitive — spinal cord injury accumulates during a long wait and may not fully reverse after surgery.
Do not wait — get evaluated now
Lumbar Spinal Stenosis
Leg pain and cramping with walking from narrowing of the spinal canal. Many patients recommended fusion for stenosis are candidates for laminectomy only — no fusion needed.
Second opinion worth having
Spondylolisthesis Grade I
Vertebral slip with stenosis. The TOPS procedure is the only motion-preserving alternative to fusion — and it is not available at any Ontario centre. If you were recommended fusion for spondylolisthesis, call first.
TOPS not available in Ontario
Degenerative Disc Disease Lumbar & Cervical
Axial back or neck pain from disc degeneration. Lumbar and cervical disc replacement preserves motion at the treated level — not available at most Ontario centres, requires device certification.
Disc replacement first
Failed Back Surgery Syndrome
Ongoing or recurrent pain after prior spine surgery. Often requires revision surgery or addressing adjacent-level disease. Second opinion before a second operation is essential.
Second opinion before revision
SI Joint Dysfunction
Sacroiliac joint pain often misdiagnosed as lumbar disc disease. Minimally invasive SI joint fusion with intraoperative CT navigation. Same-day discharge.
Same-day procedure
Recurrent Disc Herniation
Re-herniation after prior discectomy. Barricaid annular closure device reduces reherniation by 81%. If your Ontario surgeon did not offer Barricaid at your first surgery, ask whether it changes the approach for revision.
Barricaid changes the equation

Why Ontario patients travel for spine surgery

The OHIP wait is not neutral —
for some diagnoses, waiting costs function

An 18–36 month wait is a medical timeline, not just an inconvenience. For cervical myelopathy, every month of cord compression adds injury that may not recover after a technically perfect decompression. For radiculopathy, the motor recovery window closes. Waiting is not risk-free.

OHIP System — what Ontario patients are told

18–36 months for lumbar surgeryProvincial data for elective spinal surgery wait times at GTA teaching hospitals. Longer for complex cases or specific procedures.

Fusion recommended without alternatives discussedOntario spine surgeons are trained in what they perform. TOPS and disc replacement require specific device certifications most Ontario surgeons do not hold.

TOPS not available anywhere in OntarioNo Ontario centre offers TOPS. Patients with Grade I spondylolisthesis are routinely recommended fusion instead — a procedure TOPS was designed to replace.

Cervical myelopathy on a surgical wait — cord compression continuesThe spinal cord is being compressed for the duration of the wait. Early intervention produces better neurological outcomes. Urgency is clinical, not cosmetic.

Ontario patients have the right to seek care outside the province. This is your health decision. A telemedicine evaluation costs you a conversation — not a commitment.
Florida — what this practice offers

Surgery scheduled in weeks, not yearsCash-pay practice, no OHIP waitlist. From telemedicine consultation to surgery date is typically 4–8 weeks for most procedures.

Full spectrum evaluated — TOPS, disc replacement, laminectomy, fusionThe evaluation starts with what is the best option for your anatomy — not what is available locally. Many Ontario patients are fusion candidates on paper who are TOPS or disc replacement candidates in practice.

TOPS available — no fusion when motion can be preservedDr. Katsevman is on the official Premia Spine TOPS surgeon locator. The only motion-preserving alternative to fusion for Grade I spondylolisthesis available to Ontario patients.

Dr. Katsevman performs every case — incision to closureNo residents. No fellows. The surgeon you evaluate with is the surgeon who operates. Neuromonitoring on every cervical case.

Transparent cash-pay pricing: Surgical fee, facility fee, and anaesthesia quoted in full — itemised in writing before any commitment. No surprise billing. No undisclosed add-ons at discharge.

The process — from Toronto to Naples and back

How it works —
step by step for Ontario patients

Most of the evaluation happens before you leave Ontario. You do not need to be in Florida to get a thorough, independent opinion on your case.

1
Upload imaging & book telemedicine — from anywhere in Ontario

Upload your MRI, CT, X-rays, nerve conduction studies, and any prior specialist or operative reports through the practice imaging portal before your appointment. Dr. Katsevman reviews all imaging personally before the consultation — not a coordinator, not a PA. The telemedicine visit covers your diagnosis, whether surgery is indicated, what options exist for your anatomy, and a realistic recovery and travel timeline.

2
Surgical plan & transparent pricing — in writing before any commitment

Once Dr. Katsevman has reviewed your imaging and the surgical recommendation is clear, complete pricing is provided in writing before you make any decision — surgical fee, facility fee, and anaesthesia, itemised, with no surprises and no add-ons at billing. You decide whether to proceed with full cost information in hand. No deposit, no pressure, no commitment until you are ready.

3
Pre-operative workup — done in Ontario before you fly

Pre-operative blood work, cardiac clearance, and any additional imaging required before surgery are completed at your GP or local hospital in Ontario before you fly. For most procedures, arriving the day before surgery is sufficient.

4
Fly YYZ → RSW — direct, approximately 2.5 hours

Direct flights from Toronto Pearson (YYZ) to Southwest Florida International (RSW, Fort Myers) on Air Canada and Porter. RSW is approximately 30 minutes from both the Fort Myers and Naples offices. An in-person pre-operative visit with Dr. Katsevman is scheduled the day before or morning of surgery. Most procedures are same-day discharge.

5
Recover & fly home — all follow-up by telemedicine from Ontario

Most minimally invasive procedures allow flying home within days to 1–2 weeks. All follow-up is by telemedicine from Ontario — no requirement to return to Florida. A full operative report is sent to your Ontario GP. A standardized physiotherapy protocol is provided that any Ontario physio can implement. Wound checks and clinical questions handled remotely.

Recovery in Naples, Florida

You're not going home
to a waiting room.

Recovery time in Southwest Florida depends on the procedure — from flying home the next day after a minimally invasive decompression to 1–2 weeks for more involved procedures. Either way, that window is a recovery advantage, not downtime. Ontario patients arrive in January and leave in better shape than they arrived, in more ways than one.

Southwest Florida in winter

Naples and Fort Myers in January through March: 75°F, Gulf beaches, no snow. Recovery in a warm, low-stress environment measurably improves outcomes. The contrast with a Toronto winter is not incidental.

Post-op follow-up by telemedicine

Once you fly home, Dr. Katsevman continues post-operative follow-up via telemedicine from Ontario — no need to return for routine check-ins. The same direct access you had before surgery continues through your full recovery.

The total package: Fellowship-trained surgeon · motion-preserving technology · no residents · transparent pricing · advanced recovery centre · Gulf Coast winter sunshine · telemedicine follow-up from home. Ontario patients are not just buying surgery — they are buying a better recovery than the OHIP system offers.

A point that matters specifically to Ontario patients

Dr. Katsevman performs every case himself —
not a resident at Toronto Western or Sunnybrook

Every major spine surgery centre in the GTA — Toronto Western, Sunnybrook, St. Michael’s, Hamilton General — is a teaching hospital. Residents and fellows perform significant portions of every case. This is how surgeons are trained. It is not what most patients are told when they consent.

GTA academic centre — vs. what you get here

Typical Ontario academic teaching hospital · vs. this practice

At Toronto Western, a UofT neurosurgery resident may perform your laminectomy exposure, place pedicle screws, and close the wound under attending supervision. At Sunnybrook, a spine fellow may perform the majority of your fusion while the attending scrubs in for key steps. None of this is hidden — it is in the consent form if you read it carefully. But it is not what patients picture when they agree to have surgery. Here, Dr. Katsevman is the only surgeon. Incision to closure. Every case.

GTA academic centre — what typically happens
Attending spine surgeon sees you in a busy clinic; resident or PA manages most of the visit
Resident or fellow performs surgical exposure, instrumentation, and closure under attending supervision
Attending scrubs in for critical steps; extent of involvement varies by case and trainee level
Post-operative ward care managed by residents; attending available by pager
Follow-up may be with a different attending or fellow if clinic schedules change
This practice — what actually happens
Dr. Katsevman conducts every consultation personally — telemedicine or in-person
Dr. Katsevman performs every step of every operation — incision to closure, no exceptions
No residents. No fellows. The surgeon you chose is the surgeon who operates.
Post-operative communication direct with Dr. Katsevman — not through a resident service
Continuity from first telemedicine consultation through surgery through Ontario follow-up

Questions Ontario patients ask before booking

What GTA patients want to know before making the trip

My Ontario surgeon recommended fusion. Is a second opinion worth the telemedicine call?
+

For the majority of Ontario patients who contact this practice having been recommended fusion, the telemedicine evaluation finds one of three things: (1) the patient is a candidate for disc replacement or TOPS instead, with substantially better long-term outcomes than fusion for their anatomy; (2) fusion is genuinely the right answer, but a minimally invasive anterior or lateral approach would be more appropriate than what was proposed; or (3) the patient’s presenting symptoms do not yet meet the threshold for surgery and conservative management should continue. The call takes approximately 45 minutes and covers your imaging in full detail. It does not commit you to anything.

What does the pricing include? Are there hidden costs?
+

Once the surgical plan is confirmed, you receive an itemised quote in writing that includes the surgical fee, facility fee, and anaesthesia. This is the price you pay. There are no add-ons at billing, no facility surcharges discovered at discharge, and no unbundled fees. The quote is provided before you make any commitment — you decide whether to proceed with full cost information in hand. The practice does not accept Ontario health insurance; this is a cash-pay arrangement and the pricing reflects that structure.

Is it safe to fly back to Toronto after spine surgery?
+

For most minimally invasive procedures at this practice — disc replacement, laminectomy, discectomy, TOPS — flying home within a few days to 2 weeks is routinely done without complications. The specific clearance depends on your procedure and your recovery — Dr. Katsevman provides individual air travel clearance at discharge. For the ~2.5 hour YYZ flight, most patients tolerate this well after minimally invasive procedures. DVT prophylaxis (compression stockings, hydration, movement) is included in discharge instructions for all post-surgical flights. For fusion procedures requiring longer recovery, the return flight is planned accordingly.

What does my Ontario GP need to know, and will they have the records?
+

A full operative report, discharge summary, and post-operative protocol are provided at discharge and sent to your Ontario GP and any relevant specialists. Your GP has everything needed to manage routine follow-up: the procedure performed, the implants used with device identifiers, post-operative restrictions, the physiotherapy protocol, and signs of complications. Follow-up visits with Dr. Katsevman are conducted by telemedicine — no return trip to Florida required for routine care.

I have myelopathy. My neurologist says I need surgery soon but the wait is 18 months. What should I do?
+

This is the scenario where the distinction between a neutral wait and a harmful one is clearest. Cervical myelopathy from cord compression is a progressive condition — spinal cord injury accumulates during a long wait and may not fully reverse after a technically successful decompression. If your neurologist believes surgery is indicated now and you are on an 18-month surgical list, a telemedicine second opinion to determine whether your imaging and clinical picture support an earlier surgical timeline is worth having promptly, not in six months. Upload your MRI and contact the practice for an expedited evaluation.

How far is Southwest Florida from Toronto — really?
+

The direct Air Canada flight from Toronto Pearson (YYZ) to Southwest Florida International (RSW, Fort Myers) is approximately 2.5 hours. RSW is 30 minutes from the Fort Myers office and 35–45 minutes from the Naples office. From Mississauga or Brampton, you can be at the surgeon’s desk within 4–5 hours door to door — comparable in total travel time to flying to Vancouver or Calgary. Most patients describe the logistics as significantly easier than they anticipated. Porter Airlines also operates seasonal direct service during the October–April snowbird season, with fares that are often competitive.

“Most Ontario patients I see were never told disc replacement or TOPS existed. Their surgeon was trained in fusion, performs fusion well, and recommends what they know. The question is whether the patient should have access to a broader conversation about their options before committing to a procedure they will live with for decades. The telemedicine consultation exists for exactly that reason.”
