TOPS — the Premia Spine device — is headquartered in Netanya and manufactured there. TOPS is available at a small number of Israeli medical centres. If your Israeli surgeon is one of the Premia-listed providers, TOPS may already be an option locally. TOPS was conceived, developed, and is manufactured in Israel. Premia Spine’s production facility is in Netanya. The device received FDA approval in June 2023 following a pivotal US randomised controlled trial. Premia maintains a list of Israeli medical centres offering TOPS — if your surgeon is on that list, TOPS may be available to you without travelling to Florida.

What this means for Israeli patients considering a consultation here: The consultation with Dr. Katsevman is still valuable in two scenarios. First, if you have been told fusion is the only option and are not sure whether your surgeon is TOPS-certified — the consultation independently confirms whether your anatomy is a TOPS candidate and whether a local certified surgeon is appropriate. What is Barricaid and why isn’t it available in Israel?

Barricaid is an annular closure device implanted at the time of disc surgery (discectomy) to seal the defect left in the outer ring of the disc. Reherniation — the disc material recurring through the same opening — is the most common reason patients return to surgery after discectomy. Barricaid reduces reherniation risk by 81% in eligible patients (those with a large annular defect at the time of surgery). It is not offered in Israeli spine surgery simply because it has not been adopted into standard Israeli practice. If you are scheduled for a discectomy in Israel, a consultation establishes whether your defect size makes you a Barricaid candidate — and whether that benefit is worth the trip.

Will my Israeli health insurance (Kupat Cholim, Maccabi, Meuhedet) cover treatment in the USA?

Standard Israeli Kupat Cholim plans — Clalit, Maccabi, Meuhedet, Leumit — do not cover planned elective treatment abroad under the standard basket of services.
